Question: would the problem be caused by a bad or improperly installed U4 (ESD diode)?


I doubt that the ESD is the issue since you are not experiencing any issues on receive.

I'm putting out a call for someone who can replace the PCM3060 without wrecking the board, and without setting fire to their house. If I used a heat gun here, my fire insurance would double.

I'm not hearing any evidence that the problem is anything except a bad (or severely traumatized) PCM3060. I observed all the ESD precautions, but I may have shocked it. Who knows. The other option is demonic possession, and that only happened with mercury vapor rectifier tubes in the Golden Age.
